This online booking process is designed for generally healthy adults scheduling a screening colonoscopy procedure without a consultation visit.

Step 1: You’ll Be Redirected to Our Secure Scheduling System

 

You’ll be redirected to our trusted scheduling platform, GutChek.

  • Choose a date and time that works for you

    • Please note that dates will be added on a rolling basis every month

  • Carefully review the surgery center location for that specific date

    (Locations may vary depending on availability)

Step 2: Complete a Medical Questionnaire

 

You’ll be asked to complete a medical questionnaire to request your screening colonoscopy.

 

This helps us understand:

  • Your overall health including any prior procedures or relevant history

  • Medications you take

As part of the questionnaire, you will need to upload:

  • A photo of your driver’s license

  • A photo of your insurance card (front and back)

🕒 Most patients finish the questionnaire in 10-15 minutes.

Step 3: Medical Review & Scheduling Confirmation

 

After you submit your request:

  • Dr. Shah will personally review your medical information

  • Our team will confirm:

    • Eligibility for a screening colonoscopy

    • Insurance coverage

    • Correct timing and location

Step 4: Final Confirmation & Preparation Instructions

Once your request is approved:

  • Your appointment is officially confirmed

  • You’ll receive a confirmation email and detailed colonoscopy preparation instructions

  • You will show up for your procedure on your scheduled date and time.
